This is an online privacy policy (herein referred to as “Policy”) of the website https://jotno.net (herein referred to as “JOTNO Website(s)”) of Square Health Limited, a private limited company incorporated under the Laws of Bangladesh, having its office at Square Centre, 48 Mohakhali Commercial Area, Dhaka-1212, Bangladesh. JOTNO App is an online platform / mobile application of Square Health Limited. Square Health Limited is healthcare services aggregator and provide services to the Service Receiver (Patients/Customers/Consumers etc.) through the Service Provider (Doctors/Hospitals/clinic/institutions etc.).

The Service Receiver and/or the Service Provider shall be referred to as “you”, “your” or “yours” as the context may necessitate. JOTNO Website(s) and/or JOTNO App shall be referred to collectively or separately as JOTNO.

As we, Square Health Limited, we will work with data and information of a personal nature, we are sensitive to the scenario. By using our JOTNO, you require to provide Personal Information (defined hereinafter) with us.

You consent to the terms & conditions of the Policy and to our processing of your Personal Information for the purposes explained in this Policy.

This Policy applies to the collection, use, storage, processing, disclosure, and transfer of the Personal Information of you. The Personal Information collected and maintained by us shall be subject to the provision of this Policy.

The information that you have shared on our JOTNO is stored in a secure server with encryption and can be accessed only for official and service purposes.

You agree, at your own discretion, to give your informed consent prior to using the JOTNO. After receiving the informed consent from you, we shall process and use the Personal Information of you. The Personal Information will be kept private and stored securely on our JOTNO. However, we take no responsibility of loss, misuse or mishandling of the Personal Information.

• REGISTRATION

Upon signing up on our JOTNO, you are required to provide us with certain Personal Information which is reasonably necessary to provide requested services. We assume all the information provided by you is true, accurate, current, non-misleading, consistent and relevant. Once the registration process is completed, you will get login credential & password for signing in JOTNO.

Personal Information means and includes all information that can be linked to a specific individual or to identify any individual, such as:

1. Your full name, address, e-mail address, mobile number, date of birth and any proof of your identity and/or address that we may request.
2. Details of any services that you avail through our JOTNO;

4. Survey that you complete through the JOTNO or based on our request;
5. Your participation in any promotion sponsored by us;
6. Calls that we make to you or you make to us;
7. Information collected through Cookies. You may set your browser to block all cookies, including cookies associated with us. However, the Services on JOTNO may not function properly in case the cookies are disabled;
8. Your IP address, log-in times, operating system and browser type;
9. Details of your visits to our JOTNO including, but not limited to, Traffic Data, location data, weblogs and other communication data, whether this is required for our own billing purposes or otherwise and the resources that you access whilst visiting our JOTNO.

We collect the Personal Information to enable you to login and register on our app. We will not disclose, subject to conditions of this Privacy Policy, the Personal Information to any third party. We use Personal Information including but not limited to the following purposes:

We may require, under applicable laws, to retain certain records of the Personal Information and transaction history for a period as suggested by regulatory authorities even after closure of our JOTNO.

We may share your Personal Information of the Service Receiver with the Service Provider who may download/use such medical records for consultation purpose or to provide you the services that we offer through JOTNO, conduct quality assurance testing, facilitate creation of accounts, provide technical and customer support, or provide specific services, such as synchronization of your contacts with other software applications, in accordance with your instructions;
2. We may share your personal information with our subsidiaries, joint ventures, or other companies under a common control that we may have now or in the future, in which case we will require them to honor this Privacy Policy. If another company acquires our company or our assets, that company will possess your personal information, and will assume the rights and obligations with respect to that information as described in this Privacy Policy;
3. We may disclose your Personal Information to third parties and/or regulatory authority in a good faith belief that such disclosure is reasonably necessary to:

   • ii. Enforce or apply our Terms and Conditions and Privacy Policy; and

4. We may pass the Service Receiver details to agents and subcontractors to help with analyzing data and providing marketing or customer service assistance. However, certain third-party service providers, such as payment gateways and other payment transaction processors, have their own privacy policies. At this specific services from the third-party service provider, we shall have to provide the information to them for the services;
5. In case of revelation of such sensitive personal data to any third party, we shall under no circumstances be held liable for any indirect, incidental, special or consequential damages caused to you due to loss of your Personal Information.
• CONSEQUENCES OF NOT PROVIDING PERSONAL INFORMATION

If you choose not to provide your Personal Information that is mandatory to process your request, we may not be able to provide the corresponding services.

• SERVER LOGS

In order to ensure easy and comfortable surfing on our JOTNO, each time you visit our JOTNO, the computer collects certain statistical information. These statistics are only used to provide us information in relation to the type of Service Receiver using our JOTNO by maintaining a history of page requests and at no point do they identify the personal details of the Service Receiver. We may make use of this data to understand as to how our JOTNO is being used.

• PAYMENTS AND BANK TRANSACTION SAFETY
1. At the time of making payment you require to enter sensitive personal data such as Bank account details, credit card or debit card details, or any other payment instrument details for registration, the said information will be protected by the terms & conditions of the payment service provider;
2. You shall be solely responsible for providing above-mentioned information for making such transactions. We take no responsibility or liability for the improper use of any information relating to the usage of credit cards/debit cards.
